New Patient Checklist
- Complete your secure online new patient form. This comprehensive medical history form is essential and will be reviewed by your provider prior to your visit. Please complete your New Patient form here.
- Request and forward your previous dental records. We have found the easiest method to request records is to email your previous dentist and copy us at frontdesk@fergusondental.com. We also have a records release form on our website if you prefer. Please be sure to request bitewing x-rays from the past 2 years along with full mouth or panoramic x-rays from the past 5 years. X-rays are a vital aid in the assessment and diagnosis of your oral health. Providing previous records eliminates needless and repetitive dental x-rays.
- Please arrive for your appointment a few minutes before you are scheduled. We strive to run on-schedule and you can and should expect to be seated in a treatment room on-time. We are located at the corner of Gordonhurst Avenue and Valley Road. The entrance to our parking lot is on Valley Road.
